Neurosurgery case - carotid endarterectomy
Suguna Hospital treated a 49-year-old man with type 2 diabetes who experienced sudden giddiness and loss of consciousness for 2-3 minutes. Upon evaluation, the patient had high blood pressure (210/100mmHg) but was neurologically intact with no deficits. Further testing revealed a cerebrovascular event, with acute infarcts in the left parieto-occipital lobe/PCA-MCA and left temporal lobe and caudate nucleus. The patient had a large fibro-fatty plaque in the left common carotid artery, causing 90-95% diameter stenosis. CT-Carotid and cerebral angiogram showed thrombosis of the left distal common carotid artery, causing 80-90% diameter stenosis for a length of 2.8cm.
The hospital managed the case in the ICU with utmost care to prevent further episodes of embolic showers-related events. A vascular surgeon was involved and an urgent left carotid endarterectomy with thrombectomy and patch angioplasty was performed. Post-operatively, the patient was stabilized and walked out of the hospital with no neurological deficits.
